What is Medicare Supplemental Insurance?

Original Medicare covers a lot — but not everything. When Medicare pays its share of your medical bills, you're typically left responsible for deductibles, coinsurance, and copayments that can add up quickly. Medicare Supplement insurance, also known as Medigap, is designed to cover those remaining costs so you're not caught off guard by an unexpected bill.

The basics.

What Medicare Supplement insurance is — and what it does

Medicare Supplement plans are private insurance policies sold by companies licensed to offer Medicare products. They work alongside Original Medicare — not instead of it. When you receive a covered service, Medicare pays its portion first, and your Supplement plan picks up some or all of what's left.

The out-of-pocket costs a Supplement plan can cover include your Part A and Part B deductibles, coinsurance, copayments, and hospital costs that continue after Medicare stops paying. Depending on the plan you choose, it may also cover costs at a skilled nursing facility and, in some cases, emergency medical care while traveling outside the United States.

For people who want predictable healthcare costs and the freedom to see any Medicare-accepting provider without worrying about what they'll owe afterward, a Medigap plan is often the most straightforward path to peace of mind.

Eligibility.

Who can purchase a Medicare Supplement plan

To buy a Medigap policy, you must already be enrolled in both Medicare Part A and Part B. Medicare Supplement plans are not a replacement for Original Medicare — they are an addition to it. If you haven't yet signed up for Parts A and B, you'll need to do that first before a Supplement plan can be purchased.

It's also important to know that Medicare Supplement plans cover one person only. If you and your spouse both want Medigap coverage, each of you will need your own separate policy. Spouses cannot share a single plan.

Medicare Supplement plans do not include prescription drug coverage. If you need Part D coverage for your medications, you'll add a standalone Part D drug plan separately alongside your Medigap policy.

Timing matters.

Your Medigap Open Enrollment Period — and why it matters

When you first enroll in Medicare Part B and are 65 or older, you receive a six-month Medigap Open Enrollment Period. This window begins on the first day of the month your Part B coverage starts, and it is the most important enrollment opportunity you'll have.

During your Open Enrollment Period, insurance companies cannot deny you coverage, charge you more, or turn you away due to any pre-existing health conditions. You have your choice of any Medigap plan available in your state, regardless of your medical history. This is your one guaranteed chance to get into a Supplement plan without health underwriting.

Once this six-month window closes, it does not come back. Unlike the Medicare Annual Enrollment Period in the fall — which applies to Part D drug plans and Medicare Advantage plans — the Medigap Open Enrollment Period is a one-time opportunity. After it passes, insurance companies in most states can review your health history and decline your application or charge you a higher premium based on your health conditions.

This is one of the most consequential timing decisions people face when first entering Medicare. Enrolling in a Medigap plan during your Open Enrollment Period gives you protections that may not be available to you later.

Standardized coverage.

The 10 Medigap plans — and how to think about them

Medicare Supplement plans are standardized by the federal government. There are 10 different plan types, each identified by a letter: A, B, C, D, F, G, K, L, M, and N. Every insurance company that sells a particular plan letter must offer the exact same benefits for that letter — so a Plan G from one company covers the same services as a Plan G from any other company. The only difference between companies offering the same plan letter is price.

This standardization makes comparison shopping straightforward once you decide which plan letter fits your needs. You don't have to worry about one company's Plan G being better or worse than another's — you're comparing premiums, not benefits. Rates can vary significantly between carriers for the same coverage, so it's worth getting quotes from multiple companies.

Most people choose from three plans that offer the broadest coverage:

  • Plan F — the most comprehensive option, covering all Medicare cost-sharing and leaving you with zero out-of-pocket on covered services. Available only to those who became eligible for Medicare before January 1, 2020.

  • Plan G — identical to Plan F except you pay the Part B deductible once per year. Often results in lower premiums than Plan F, sometimes with overall annual savings.

  • Plan N — covers most costs but includes small copays for some office visits and emergency room visits. Lower premiums in exchange for that modest cost-sharing.

There is no single right answer. The best plan depends on how often you use healthcare services, how much predictability you want in your costs, and what premium fits your budget. Some people prefer to pay a higher premium for complete coverage; others prefer to keep premiums lower and pay a small amount at the time of service.

Beyond filling the financial gaps in Original Medicare, Medigap plans offer several practical advantages that make them especially appealing for people who value flexibility and simplicity in their healthcare.

  • See any doctor or specialist who accepts Medicare — no referrals required

  • Use your plan anywhere in the United States, not just in a local network

  • Predictable out-of-pocket costs throughout the year

  • Guaranteed renewability — your insurer cannot drop you or change your coverage because of your health

  • No annual lock-in — you can drop your supplement at any time

These features make Medicare Supplement plans particularly well-suited for people who travel frequently, split time between states, see multiple specialists, or simply want to avoid the uncertainty of variable costs each time they access care.

Know your rights.

Every Medicare Supplement policy must follow federal and state laws designed to protect you as a consumer. Insurance companies are required to clearly disclose the benefits of each plan, cannot misrepresent coverage, and must honor the standardization rules that make each plan letter consistent across carriers.

Be cautious when shopping. There are known illegal practices in the Medicare insurance market — including misleading marketing, pressure tactics, and misrepresentation of coverage. If something feels off during the sales process, trust that instinct. You are never required to make a decision on the spot, and a reputable agent will not pressure you to do so.
